LPN Position at Cape Heritage Rehabilitation & Health Care Center

Cape Heritage Rehabilitation & Health Care Center in Sandwich offers up to a $5,000 sign-on bonus available! Hourly rate ranges between $40 and $46, depending on the shift. Evenings, overnights, and weekends have shift differentials.

Full-time and part-time available on 11p-7a; includes an every-other-weekend rotation. Per diem available on all shifts.

Job Summary:

As an LPN, you will provide direct nursing care under the supervision of the RN to residents and supervise the day-to-day nursing activities performed by the nursing assistant to ensure that the highest degree of quality care is maintained at all times.

Responsibilities:

• Meet the nursing and rehabilitation needs of each resident
• Maintain current resident care plans on all residents under your care
• Follow physician's orders and render/assist with complete nursing care, treatments, dressings, and other physician orders
• Transcribe physician's orders promptly and accurately
• Review patient records daily to assure accuracy and completeness
• Document comprehensive and complete nursing notes
• Visit each resident daily to evaluate resident needs
• Maintain accurate awareness of residents on alert list
• Document and report any unusual or significant findings; contact the physician when necessary
• Administer medication according to procedure
• Initiate any and all emergency procedures as necessary
• Demonstrate correct safety techniques
• Respect and take care of facility equipment and not wasteful of supplies
• Contribute to effective communication and pleasant working conditions
• Contribute to and promote positive resident and family relations
• Comply with facility Privacy Practices Policies & Procedures, protecting residents' individual health information

Requirements:

• Graduate of an approved LPN/LVN program
• CPR certified
• Active and unencumbered LPN/LVN license in Massachusetts, or be in the process of obtaining one
• New graduates welcome!

Additional Requirements:

• Ability to read, write, speak, and understand the English language
• Ability to make independent decisions when circumstances warrant such actions
• Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public
• Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care
• Leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel
