Physical Therapist

Schedule: M-F, Days; Rotating weekend coverage

Assignment Details:

• Guaranteed Hours: 40
• Contract Length: 13 weeks

Requirements:

• Current Professional Licensure Required
• Current IL Physical Therapist License Required
• Valid Driver's license and proof of personal auto insurance Required
• BLS Required
• APTA Membership Preferred
• Physical Therapy minimum 1 year Preferred
• Strong computer skills including Microsoft Word, Excel and Outlook

Roles & Responsibilities:

• Evaluate, plan, direct, and administer Physical Therapy services as prescribed by a licensed Provider.
• Treat patients using Physical Therapy modalities and skilled therapy techniques; instruct patients on the use of adaptive/assistive equipment.
• Supervise and direct Physical Therapy Assistants; maintain open communication with members of the Rehabilitation team.
• Perform appropriate assessments on all patients; interpret results within scope of licensure.
• Design treatment programs for patients with various disabilities and medical problems.
• Formulate realistic short and long-term treatment goals in collaboration with the patient and family.
• Ensure accurate, timely documentation and compliance with regulatory standards.

Compliance / Notes:

• Comply with all established safety procedures to ensure a safe environment for patients, visitors, and staff.
• Participate in performance improvement activities.
