Qualifications/Experience:

• Must have a current unencumbered NYS license as a Registered Professional Occupational Therapist, Bachelor’s degree or higher.
• Graduate of an accredited school of occupational therapy or international equivalent
• Geriatric experience preferred
• A current unrestricted driver’s license and dependable transportation
• Ability to professionally communicate with internal and external resources
• Ability to function independently as well as part of a team
• Ability to function effectively in stressful and changing situations
• Ability to positively give and receive oral and written direction

Essential Job Functions:

• Evaluate residents for Occupational Therapy services upon physician order
• Develop and implement individualized treatment plans
• Provide therapeutic interventions and adjust care based on resident progress
• Document progress, treatment plans, and outcomes in accordance with regulations
• Recommend discharge from therapy services when appropriate
• Collaborate with nursing and interdisciplinary teams on mobility, ADLs, and care planning
• Participate in discharge planning and care plan meetings
• Educate residents, families, and staff on therapy-related needs
• Supervise Occupational Therapy Assistants and support staff
• Assist with equipment recommendations and procurement
• Ensure compliance with Medicare, Medicaid, and regulatory standards
• Maintain productivity and quality expectations

What We’re Looking For

• Current, unrestricted New York State Occupational Therapist license
• Bachelor’s degree or higher in Occupational Therapy
• Graduate of an accredited OT program (or equivalent)
• Geriatric experience preferred
• Valid driver’s license with reliable transportation
• Strong communication and teamwork skills
• Ability to work independently and adapt in a fast-paced environment

• Uphold resident rights, confidentiality, and safety standards
• Follow infection control and sanitation protocols
• Report concerns related to abuse, neglect, or safety immediately
• Maintain strong communication with the Rehabilitation Manager and team
• Promote a positive, collaborative work environment
