Position Overview:
The Coordinator of Continuous Quality Improvement (CQI Coordinator) is responsible for various aspects of the Continuous Quality Improvement Program, under the direction of the Associate Executive Director ("Compliance Officer") for Continuous Quality Improvement (AED for CQI). This includes but is not limited to completing tasks and duties as assigned by the AED for CQI that relate to the organizational CQI efforts and CQI committee work. This may involve record reviews to ensure documentation and services are meeting agency policy and procedure and the regulations of state oversight agencies and moreover exceed quality expectations and goals. The CQI Coordinator will make recommendations for improvement.
The CQI Coordinator facilitates all investigations of allegations of abuse and neglect and significant incidents involving incidents that occur under the auspices of St Catherine's Center for Children and will work with the state oversight agencies to render safety plans, coordinate interviews and gather information, keep administration and program staff up to date on status, work to resolution and implementation of accepted corrective action plans (CAP).
Position Requirements:
- Ability to work: Monday-Friday, 8:00am 4:00pm w/ flexibility as needed.
- Bachelor's degree required; Associate's degree with applicable experience in a related field may be considered.
- Knowledge of Medicaid regulations; Knowledge of the New York Foster Care system specific to Voluntary Foster Care Agencies regulated by the Office Children and Family Services; Children's Day Treatment Clinics regulated by the Office of Mental Health; CFTSS programs also regulated by the Office of Mental Health; the HCBS & Health Homes programs regulated by the Dept. of Health; and the R&E May School as regulated by the State Education Department.
- Local travel is occasionally required; A Clean and Valid New York State Driver's License (actively licensed at least one year) is required *.
- Experience completing investigations; Justice Center experience is highly preferred.
- Proficiency in various computer applications: Microsoft Office Suite (Word, Excel), Google Business Workspace Suite (Gmail, Google Docs, Meet, Sheets, etc.); and use secure Internet practices.
- Prior experience with FootHold Technology, Awards and ExtendedReach, is a plus!
- Outstanding interpersonal and communication skills; Capable of building strong working relationships with regulatory bodies.
- Self-motivated with a strong attention to detail is a must!
- Capable of exercising ethical, independent judgment and discretion, while handling sensitive data with strict confidentiality; and capable of complying with all HIPAA requirements is highly essential.
*Staff in driving positions must be able to meet the minimum requirements of the agency's insurance policy for authorized drivers; including, but not limited to the minimum age of 21 at time of hire.
Essential Duties & Responsibilities :
- Assist the AED for CQI in the development, revision and implementation of agency policies and procedures and work with the CQI committee on policy development.
- Develop an understanding and maintain up-to-date knowledge of standards, policies, regulations and mandates set by: St. Catherine's Center for Children (St. Catherine's), the New York State Office of Child and Family Services (OCFS), the New York State Education Department (SED), the New York State Office of Mental Health (OMH), the Office of Temporary and Disability Assistance Services (OTDA), the Department of Health (DOH), and/or other regulatory bodies.
- Assist the AED for CQI with the Council of Accreditation (COA) standards and reaccreditation process as requested.
- Ensure CQI standards are met as defined by the Quality Improvement Plan goal process as well as adherence to agency and regulatory policy and procedures/compliance standards by conducting the following:
- Review of case records (both paper and electronic) for Quality and Compliance.
- Coordinating with program leadership to form a corrective action response, following case record & program reviews;
- Regular and ongoing policy and procedural reviews to include requesting updates and modifying, as needed.
- Coordinate investigative processes involving regulatory bodies and State Oversight agencies.
- Maintain all institutional (IAB) reports and communications and ensure compliance with standards and timelines.
- Facilitate all investigations involving regulatory bodies while collaborating with involved programs to ensure a timely completion of the investigation; including maintaining contact with investigators and ensuring all necessary documentation is provided.
- Receive and Review all reports of significant incidents; provide additional information as needed.
- Maintain a tracking system of all investigations and significant incidents annually.
- Assist in the development of any needed performance improvement plans (PIP) and provide copies of the PIP with supporting documentation to regulatory bodies; Maintain all documentation regarding completed actions in a PIP.
- Develop knowledge of Foothold Technology/ Awards Electronic Health Record (EHR) and how to be proficient in its use.
- Assist the AED for CQI with implementing our compliance program and ensuring its effectiveness.
- Participate as an active member of the Agency Medicaid Compliance Committee.
- Additional position-related duties and responsibilities as assigned by the AED for CQI.
