Qualifications: They shall have at high school diploma or GED and at least 5 years of experience working with individuals with developmental disabilities. Experience writing reports, individual plan goals, and yearly summaries, etc., is preferred. A valid Missouri chauffeur's license is required, along with the ability to work effectively as a team member and maintain a professional demeanor at all times. Summary of Duties: This role involves writing, implementing, and overseeing individual program plans for an assigned caseload of individuals. Close collaboration with other team members to ensure effective delivery of services is required. This role will oversee the health and safety of individuals across all environments. An individual in this role is expected to be a team leader and role model, guiding and supervising direct support professionals as needed. Becoming a Tools of Choice Facilitator and developing proficiency in Charting the Life Course is required. Additionally, flexibility is essential, as responsibilities may vary to accommodate program needs. The overall goal is to maintain an efficient, safe, and supportive environment for the individuals being served. Interpersonal Skills:

- CPR/First Aid
- Nonviolent Crisis Interventions
- Level 1 Medication Aid Certification
- Tools of Choice
- Mo Class E Chauffer's license
- Ability to walk, stand, bend, stoop, kneel, and reach for extended periods throughout the workday
- Capability to lift, transfer, push, pull, or carry up to 50 pounds independently, and assist with heavier loads using proper equipment or team lifting techniques.
- Physical ability to safely assist individuals with mobility needs, including transfers to/from wheelchairs, vehicles, beds, and seating
- Ability to navigate stairs, uneven terrain, and other environmental barriers while supporting individuals as needed
- Sufficient strength, coordination, and stamina to perform personal care tasks (e.g., bathing, dressing, toileting assistance).
- Demonstrates fine and gross motor skills necessary for daily caregiving duties, documentation, and safe operation of equipment. Including, but not limited to the use of a telephone, iPad and/or laptop
- Ability to enter and exit vehicles frequently and assist passengers safely in various environments
- Ability to operate a motor vehicle safely for extended periods, including maintaining alertness and proper posture while driving
- Physical capability to perform CPR and emergency response procedures as required
- Ability to perform light maintenance tasks, such as cleaning and washing vehicles
- Adequate vision (with or without correction) to safely drive, monitor surroundings, and recognize changes in client condition.
- Adequate hearing to communicate effectively, respond to alarms or signals, and ensure client safety
- Ability to communicate clearly (verbally and in writing) for documentation and coordination of care
- Physical and mental ability to respond quickly in emergency or high-stress situations
- Ability to pass and comply with random drug and alcohol testing requirements to ensure safety-sensitive job performance

- Establish and maintain respectful relationships with individuals served, families, and coworkers during all interactions.
- Maintains professional behavior and appropriate boundaries at all times.
- Protects confidentiality and comply with HIPAA requirements by safeguarding all personal and health information with zero unauthorized disclosures.
- Uses at least one activity or approach each shift to engage individuals served.
- Follow supervisor directives by completing assigned tasks accurately and within designated timeframes during each scheduled shift.
- Works collaboratively with co-workers, responding to team needs, and participating in shift responsibilities during all scheduled work hours.
- Shares important information with supervisors or team members on time (within the same shift when needed).
- Reports schedule changes to supervisor before the shift or as soon as possible.
- Accepts and completes additional duties as assigned
- Completes and submits all assigned reports with 100% accuracy by established deadlines (daily, weekly, monthly as assigned),
- Operates services in accordance with established program policies, DMH regulations, and CARF standards during all shifts to ensure compliance, quality service delivery, and safe living conditions.

- Attends 100% of scheduled staff meetings and required trainings (or reviews approved make up materials) to ensure consistent service delivery and compliance
- Participate in interdisciplinary team processes to complete screenings and placement, and develop PCSP goals annually, ensuring alignment with individual needs and outcomes.
- Records PCSP data daily with ≥95% accuracy and provides initial and ongoing staff training on implementation protocols within 5 business days of updates, ensuring consistency in service delivery.
- Coordinates, directs, and supervises assigned staff on a daily basis, providing coaching and documented feedback at least monthly to ensure adherence to program standards and quality care outcomes.
- Reviews and updates required files, medical records, and IP documentation at least monthly, maintaining 100% compliance with organizational, state, and federal standards.
- Collaborate with the Quality Outcome Specialist (QOS) on at least a monthly basis to assess staff training needs and monitor individual progress toward outcomes, ensuring service quality and compliance.
- Completes and submits all required paperwork related to individual benefits and services with 100% accuracy by deadlines, preventing disruptions in service eligibility or funding.
- Completes required Charting the Life Course training and facilitates the use of resources and tools with individuals and teams at least quarterly, ensuring person-centered planning and alignment with individual life outcomes.
- Completes required Tools of Choice Facilitator training and facilitate staff training module at least quarterly.
- Conducts routine checks (at least monthly) to ensure proper sanitation, maintenance, and secure use of ABiLITY property and equipment, reporting and resolving issues within 1 business day to maintain safety and operational readiness.
- Completes all assigned PCSP checklist items with 100% accuracy by required timelines (daily, monthly, or annually as applicable), ensuring compliance with regulatory standards and continuity of individualized services.
- Consistently prioritizes safety and welfare by identifying potential risks, taking immediate corrective action, and communicating concerns to management during each shift to ensure a safe and secure environment
- Apply positive behavior support strategies during all interactions with individuals served and team members to promote appropriate behaviors, enhance participation, and maintain a respectful, safe environment
