Job Summary:

The Discharge Planner is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient’s resources and right to self-determination. This role involves collaborating with healthcare providers, patients, and families to create effective continuing care and discharge plans. Key responsibilities include coordinating referrals, follow-up appointments, and admissions to other facilities; maintaining accurate records; and ensuring smooth communication with community resources. The position requires active participation in treatment team meetings and ongoing research to keep provider and resource information up to date. This position will also provide clerical support to the Case Managers and Social Workers.

QUALIFICATIONS:

B.S. Sociology / Psychology or other related field is required with a minimum of 1 year working in a behavioral health environment.

BLS (American Heart Association) for healthcare providers upon hire and CPI certification within 30 days of hire.

Placement in the pay range is based on multiple factors, including but not limited to relevant years of experience and qualifications. In addition to base pay, additional compensation may be available for CHLB/Glendora Hospital roles, such as shift differentials and other special pay practices. The posted compensation for this position is a reasonable estimate that ranges from the lowest to the highest pay CHLB/Glendora Hospital, in good faith, believes it might pay for this job based on the circumstances at the time of posting.
