Our team and state-of-the-art programs create a world-class health care organization and the destination center for cardiac, thoracic, and vascular care with complex structural heart disease and interventions, advanced heart failure service as well as advanced imaging with cutting edge technologies.**FTE:** 1.0, **Shift:** Days, **Schedule:** Mon-FriArea of specialty includes cardiac or critical care experience**Position Summary**The Nurse Navigator Acute Care is a multi-faceted position providing comprehensive services to patients throughout the acute care continuum working in the role of registered nurse, case manager, and patient/physician liaison. This position is responsible for the clinical, quality, and cost effective outcomes for a caseload of patients, and for collecting, trending, and analyzing aggregate outcomes data for the patient population to be used in identifying best practices and targeting performance improvement efforts. This position requires the advanced knowledge and expertise to manage cardiovascular hospitalized patients, understand triage principles, and identify and implement improvement processes. Work situations require substantial clinical competency and specialized knowledge, judgment, and skill based upon the physical, developmental, psychological, environmental and spiritual principles. This position requires self-direction, motivation, organization, and interpretation of information relating to policies, procedures, and programs. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $121,118.40 - $197,579.20 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education.
