Responsibilities:

• Collaborate with the care team to complete and maintain accurate Minimum Data Set (MDS) assessments for residents
• Develop and implement comprehensive care plans that meet the unique needs of each resident
• Conduct regular assessments and evaluations to monitor resident progress and adjust care plans as necessary
• Communicate effectively with residents, families, and healthcare professionals to ensure seamless continuity of care
• Maintain accurate and detailed records of resident assessments, care plans, and progress
• Participate in quality improvement initiatives to improve resident outcomes and satisfaction

Qualifications:

• Active and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license, as required by state regulations.

• Previous experience in MDS, long-term care, skilled nursing, or a related healthcare setting preferred.

• Knowledge of the Minimum Data Set (MDS) assessment process, RAI Manual, and applicable CMS regulations.

• Strong understanding of Medicare, Medicaid, and reimbursement guidelines.

• Excellent assessment, clinical documentation, and critical thinking skills.
