MDS Specialist
We are seeking a dedicated and skilled MDS Specialist to join our team at Oasis at the Conch Republic in Key West, Florida. As a vital member of our care team, the MDS Specialist will play a crucial role in ensuring our residents receive the highest level of care and attention. Responsibilities:
• Collaborate with the care team to complete and maintain accurate Minimum Data Set (MDS) assessments for residents
• Develop and implement comprehensive care plans that meet the unique needs of each resident
• Conduct regular assessments and evaluations to monitor resident progress and adjust care plans as necessary
• Communicate effectively with residents, families, and healthcare professionals to ensure seamless continuity of care
• Maintain accurate and detailed records of resident assessments, care plans, and progress
• Participate in quality improvement initiatives to improve resident outcomes and satisfaction

Qualifications:
Active and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license, as required by state regulations.
Previous experience in MDS, long-term care, skilled nursing, or a related healthcare setting preferred.
Knowledge of the Minimum Data Set (MDS) assessment process, RAI Manual, and applicable CMS regulations.
Strong understanding of Medicare, Medicaid, and reimbursement guidelines.
Excellent assessment, clinical documentation, and critical thinking skills.
