Summary Home-Based Primary Care RN for HCC (Fredericksburg) positions located at the Fredericksburg Health Care Center in Fredericksburg, Virginia. This role provides direct primary care to homebound Veterans, coordinating care with an interdisciplinary team. Qualifications English language proficiency: must be competent in basic written and spoken English. Graduate of a nursing school approved by an accrediting agency (ACEN or CCNE) or equivalent. Master's level bridge program students may become licensed before completion and be appointed temporarily, later permanently upon graduation. Foreign nursing program graduates must hold current, full, active, and unrestricted registration and, if education is higher than an associate degree, provide formal equivalency validation from a recognized evaluator such as International Consultants of Delaware. Registered Nurse with a State, Territory or Commonwealth license (including Washington D.C.) or a Graduate Nurse Technician (GNT) with a 120‑day temporary appointment while pursuing licensure. Valid State Driver's license required. Grade determination follows Scope‑Education and Dimension criteria based on practice, leadership, and evidence‑based practice. Education and experience requirements for levels I‑III (details available in standard): Nurse I‑Level I: Associate Degree or Diploma + foundational clinical experience. Nurse I‑Level II: ADN or Diploma + 1 year specialized experience or BSN without additional experience. Nurse I‑Level III: ADN or Diploma + 2 years specialized experience, or BSN + 1 year, or MSN/related master’s with no additional experience. Nurse II: BSN + 2 years or MSN + 1 year, or related master’s + BSN + 1 year. Nurse III: MSN + 2 years or related master’s + BSN + 2 years or doctoral degrees without experience. Preferred: 2‑3 years experience in home‑care or primary care outpatient setting; MSN or certifications in case management, geriatrics, or ambulatory care. Physical requirements: lifting, prolonged standing and sitting, frequent bending and stooping, walking, reaching, repetitive motion, stairs. Mental/sensory requirements: logical thinking, processing, memory, reading, recall, reasoning, complex problem solving, hearing, clear speech, writing, keyboarding, telephone and video connectivity. Duties Two full‑time positions for an experienced RN in the Geriatric Extended Care (GEC) Home‑Based Primary Care (HBPC) Program reporting to the HBPC Nurse Manager. Member of the interdisciplinary team providing direct primary care to homebound Veterans with chronic and terminal illnesses. Care coordination on an assigned caseload, implementing home health plans that comply with VA policies and regulations. Participate in weekly interdisciplinary team meetings and continuous quality improvement activities. Oversee nursing care for Veterans receiving community‑care sponsored or contract programs. Conduct comprehensive in‑home assessments, including risk factors, physical, psychosocial, emotional, mental status; medication review; home environment and safety; caregiver support; educational needs. Develop initial and ongoing care plans with patients and caregivers; coordinate referrals to team members (Provider, Social Work, OT, Dietitian, Psychologist); review plans every 90 days. Collaborate with HBPC pharmacist for medication review; consult with discharge coordinators, nurses, and social workers to facilitate transition to home care. Operate government vehicle for home visits. Compensation and benefits: Competitive salary with regular increases and potential performance awards.
