CASE MANAGER (PT) Monday - Friday 9:00 am to 3:00 pm (Paid vacation and sick time with a supplemental insurance package) SUMMARY Screens and assesses the eligibility of elderly candidates for participation in the Homebound Meal Program. ESSENTIAL DUTIES AND RESPONSIBILITIES Educates the community on various homebound meal outreach programs and the importance of utilizing the program Distributes educational materials in the community Visits homes to determine client needs, develops a plan to meet needs Identifies and monitors clients for homebound meal program Completes required paperwork in compliance with state and program regulations Creates and maintains records in SERV Tracker software Administers initial nutritional screening in the form of a questionnaire Helps client obtain necessary resources Refers clients with social and emotional problems to other community agencies for assistance Refer clients to the Director of Nutrition Services for nutrition counseling as needed Documents client cases, including assessments and reassessments, and maintains records on each client Prepares reports as required by various federal, state, and local programs Helps organize and participate in fundraisers benefiting the program COMPETENCY Gathers and analyzes information skillfully, must be computer literate Manages difficult or emotional customer situations Contributes to building a positive team spirit Writes clearly and informatively Shows respect and sensitivity for cultural differences Follows policies and procedures Is consistently at work and on time Demonstrates accuracy and thoroughness EDUCATION/EXPERIENCE Must have a high school diploma and one to two years of related experience and/or training. COMPUTER SKILLS (must know of): Microsoft Word software Microsoft Excel spreadsheet software Ability to learn specific software needed for the job, i.e., SERV Tracker CERTIFICATES AND LICENSES Valid driver's license required WORK ENVIRONMENT While performing the duties of this job, the employee is occasionally exposed to fumes or airborne particles and outdoor weather conditions PHYSICAL DEMANDS While performing the duties of the job, the employee is frequently required to walk, sit, and talk The employee must frequently lift and/or move up to 10 pounds. #J-18808-Ljbffr
