Job Description:

We are currently seeking an experienced Physical Therapist (PT) to join our team.

Position Overview:

As a Physical Therapist, you will assess and treat patients to help them regain physical function and improve mobility. Collaborating with a multidisciplinary healthcare team, you will design and implement individualized treatment plans aimed at promoting independence and enhancing the quality of life for residents.

Key Responsibilities:

• Conduct thorough evaluations to assess patients' physical abilities and develop personalized treatment plans.
• Provide physical therapy interventions to improve strength, mobility, and functional abilities.
• Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications.
• Collaborate with the interdisciplinary team, including occupational therapists, nurses, and physicians, to coordinate care and optimize patient outcomes.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
• Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in accordance with facility policies.
• Ensure care plans adhere to regulatory requirements and best practices in physical therapy.

Qualifications:

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
• Valid Physical Therapy license in the relevant state.
• Experience in a skilled nursing facility or similar healthcare setting is preferred.
• Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment.
• Excellent communication, interpersonal, and problem-solving skills.
• Ability to work independently and collaboratively in a team environment.
• Compassionate, patient-focused approach to care.
