Physical Therapist Assistant

Through our growing network of services and partners, we are building a model that reduces complexity, supports caregivers, and reimagines how at home healthcare is delivered across the U.S. Duties and Responsibilities Provides direct patient care, progress functional needs and outcomes, and consults with other specialists and the attending physical therapist. Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes in consultation with the physical therapist. Conducts therapy practice within defined standards of care. Directs the assistance of support staff to achieve patients’ functional goals. Communicates effectively with other professional and support staff to achieve positive patient outcomes. Practices departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards. Documents treatment progress, and updates plans in consultation with the physical therapist. Regularly updates documentation and maintains prescriptions and signatures. Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature. Qualifications Graduate of APTA or CAPTE approved Physical Therapy Assistant program. Basic Life Support Certification from the American Heart Association. Physical Requirements Duties require standing and walking. Requires lifting, positioning, pushing, and/or transferring patients. Duties require frequent reaching, stooping, bending, kneeling, and crouching.
