Role Overview As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes. You are a key part of the care team – delivering treatments, monitoring patient conditions, and reinforcing education to support successful outcomes.
