Provide counseling, crisis intervention, and emotional support to patients and their families. Coordinate discharge planning and connect patients with community resources, financial assistance, and rehabilitation services. Collaborate with doctors, nurses, and other healthcare professionals while maintaining accurate case documentation and advocating for patients' needs.
