Telehealth RN Case Manager

Position Overview

The Tele-Health Nurse provides high-quality service and clinical support to patients, caregivers, healthcare providers, and clinic staff regarding prescription and specialty drug programs. Operating through inbound and outbound telephonic and electronic interactions, this role ensures adherence to regulatory guidelines and program protocols while coordinating key services such as patient testing, clinical monitoring, and follow-up counseling. All responsibilities are performed in strict accordance with the Nurse Practice Act and state licensure guidelines.

Key Responsibilities

Utilize Behavior Modification and Motivational Interviewing techniques during telephonic outreach to encourage, achieve, and sustain patient medication adherence.

Coordinate the collection of patient-specific clinical data required to support therapy protocols and program regulatory compliance.

Maintain a deep understanding of, and compliance with, service line specifications and regulatory guidelines.

Apply clinical knowledge and established procedures to identify stakeholder needs, health risks, and safety concerns, ensuring timely reporting and escalation.

Conduct structured follow-ups with physicians and patients to drive therapy persistency and compliance with program protocols.

Identify and document Adverse Events (AEs) and Product Complaints per established regulatory guidelines.

Maintain active, unrestricted professional nursing licensure throughout employment.

Minimum Qualifications

Education & Licensure: Completed vocational or technical training. Current, active Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure is required and must be maintained throughout employment.

Experience: 2+ years of experience working within a healthcare company, physician practice, or laboratory setting.

Communication & Interpersonal: Superior written and verbal communication skills with the ability to articulate complex clinical and program concepts clearly, professionally, and empathetically to diverse stakeholders.

Technical Skills: Strong computer proficiency, including fast data entry/keyboarding, competence with Microsoft Office Suite, and the ability to navigate multiple web platforms and software tools simultaneously.

Core Competencies:

Proficiency in medical terminology, medical document interpretation, and procedural manual compliance.

Strong problem-solving capabilities and the ability to work independently to research agencies and resources for patient assistance.

Exceptional attention to detail, accuracy in note documentation, and regulatory data collection.

High adaptability to change and comfort operating in a fast-paced healthcare environment.
