This role offers the opportunity to be part of a specialized team dedicated to improving patients lives through high-quality, collaborative care all while enjoying a vibrant community with plenty to offer.**About the Position*** 100% Inpatient Role.* Schedule: Monday through Friday, 8:00 AM 4:30 PM.* Key responsibilities: Bridge between the hospital and clinic, get patients the appropriate care after hospitalization.
