Physical Therapist (PT) – PRN

Location: Silver Memories Health Care – Versailles, IN

Silver Memories Health Care is seeking a PRN Physical Therapist (PT) to join our team at our skilled nursing facility in Versailles, IN. This is a great opportunity for a motivated clinician to provide hands-on therapy and help residents regain strength, function, and mobility—while enjoying a flexible, part-time schedule.

What You’ll Do:

• Provide physical therapy services and develop individualized treatment plans based on residents’ needs and functional goals

• Evaluate residents and establish appropriate treatment plans in accordance with professional standards of practice

• Implement therapeutic interventions focused on improving strength, mobility, balance, endurance, and functional independence

• Monitor resident progress and modify treatment approaches as appropriate

• Document resident progress accurately and communicate effectively with residents, families, and the interdisciplinary care team

Qualifications:

• Doctorate, Master’s, or Bachelor’s degree from an accredited Physical Therapy program

• Active PT license in Indiana

• Commitment to ethical, patient-centered care

• Strong communication and interpersonal skills

• Willingness to participate in ongoing professional development
