Job TitleProvide patient care services to the Pueblo of Laguna (POL) tribal and surrounding community, within a collaborative team environment focused on the Patient Centered Medical Home Model (PCMH).Perform examinations, diagnose, treat, and help prevent diseases and injuries that commonly occur in the POL and surrounding community. May refer patients to specialists when needed for further diagnosis or treatment.Essential FunctionsPerform routine and non-routine physical examinations of patients with the evaluation of symptoms to determine appropriate diagnosis and treatment of illness, conditions' and overall health status of tribal and other community members in a culturally sensitive environment.Collect, record, and maintain patient information, such as medical history, reports, or examination results in consultation with patient, family members or other medical professionals.Order medical diagnostic or clinical tests, perform and interpret tests and analyze records, images, reports, and examination information to diagnose patients' condition.Perform minor procedures to treat skin conditions, injuries or lacerations including sutures. Remove, repair, or improve functioning of diseased or injured body parts and systems. Treat skin conditions.Provide care for women during pregnancy and childbirth.Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury. Collaborate with healthcare professionals to plan or provide treatment.Explain medical procedures and discuss test results or prescribed treatments with patients and family members.Refer patients to medical specialists, other practitioners or health resources when necessary.Coordinate treatment plans with nurses, social workers, rehabilitation therapists, pharmacists, psychologists, and other health care providers. Work with the Pueblo's behavioral program/staff in cases presenting with behavioral health problems.Monitor patients' conditions', progress or responses to treatments. and re-evaluate treatments as necessary.Advise patients and community members concerning diet, activity, hygiene, and disease prevention. Provide health and wellness advice to patients or caregivers.Train and assist residents, medical students, and other health care professionalsProvide clinical advice to plan, implement, or administer health programs or standards in the community for prevention or treatment of injury or illness. Advise communities or institutions regarding health or safety issues.Ensure diagnosis, care and treatment follow all protocols established for healthcare delivery program.Prepare official health documents and patient records with proper and ethical coding necessary for billing of all services provided.Conduct research to increase knowledge about medical issues.Ensure compliance with regulatory and legal requirements such as mandatory reporting.Stay current on developments in the field of medicine; responsible for professional practice through active participation in professional organizations and continuing education.Contribute to the overall quality of the unit's service provision by coordinating with work teams in accordance with the PCMH model and by reviewing, recommending and implementing improved policies and procedures.Minimum QualificationsCompletion of Nursing program with clinical rotation including Primary Care, Family Practice, Urgent Care or Emergency Medicine.Master of Science Degree in Nursing from an accredited college or university.Current, active, full and unrestricted license as a Nurse Practitioner from any U.S. state or the District of Columbia.Current, active board certification as required by licensure such as American Academy for Nurse Practitioner Certification Board (AANPCB)Current DEA License.Current New Mexico Controlled Substance RegistrationCurrent ACLS/PALS certification.
