Travel Therapy - Physical Therapist

Position Overview Centra Healthcare Allied is seeking a travel Physical Therapist for a contract assignment in Sacramento, California. This outpatient role requires an experienced or newly graduated Physical Therapist/ PT who will provide direct patient care and rehabilitation services as directed by a physician. Responsibilities Assess referred patients requiring rehabilitation services, evaluating medical condition, functional capabilities, limitations and potential for rehabilitation. Establish and administer a treatment program with goals determined by the patient’s capacity and tolerance under physician direction. Set realistic, achievable goals, document patient conditions and educate patients and families on appropriate physical therapy methods. Develop a plan of care for each patient and provide skilled physical therapy services/interventions per physician orders. Assess patient needs, plan, evaluate and modify care to meet goals of physical therapy interventions. Qualifications Graduate from an accredited physical therapy school. Current state license as a Registered Physical Therapist/ PT or actively in the process of licensure.
