A Case Manager RN opportunity is available at a 267-bed hospital in Fort Walton Beach, FL, offering an Advanced Primary Stroke Center, a Cancer Care Center, open-heart surgery services, and emergency care including a pediatric ER and two freestanding ERs.

The role. Reporting to the Rehab Program Director and Facility Case Management Director, the Case Manager RN supports patients and families at every stage of their care journey. In this role, you will complete psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the interdisciplinary team. You will develop and implement individualized treatment plans that reflect each patient's strengths, goals, and recovery needs, while coordinating education for patients, families, and caregivers to encourage active participation in treatment planning and family conferences. You will also guide patients and families through program orientation, covering rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.

What we're looking for.

• Associate Degree or Certificate
• Registered Nurse (RN) licensure
• 2 years of recent (within the last 3 years) acute care experience; ICU, CCU, Med-Surg, or LTAC case management experience preferred

Schedule. Days with rotating weekends, 8:00 AM to 4:30 PM.

Compensation. $34.25 - $47.10/hr plus $10,000 sign-on bonus
