Work Environment\nThis role is primarily remote, allowing for flexibility in managing your schedule to best meet the needs of clients. Interactions with clients will often occur via virtual platforms, but occasional in-person visits to our hospice center may be necessary. The standard work hours are Monday to Friday from 8:00 AM to 4:30 PM, with the expectation of occasional evenings or weekends as client needs arise. Additionally, you will be required to be on-call to assist with urgent situations that may occur outside of regular hours.
Key Responsibilities\n- \n
- Conduct comprehensive assessments to evaluate the psychosocial needs of seniors, taking into account emotional, physical, and social factors that impact their well-being. \n
- Develop personalized care plans in collaboration with clients and their families to address specific goals, preferences, and needs, ensuring a person-centered approach. \n
- Provide one-on-one counseling and emotional support to seniors, assisting them in navigating significant life transitions such as loss, illness, or adjustments to living conditions. \n
- Advocate on behalf of clients to ensure access to necessary community resources, services, and supports that enhance their quality of life. \n
- Work collaboratively with an interdisciplinary care team, including healthcare professionals, nurses, and caregivers, to deliver a holistic approach to client care. \n
- Conduct regular follow-ups and site visits, monitoring clients' well-being, living conditions, and any safety concerns that may arise. \n
- Facilitate effective communication among family members and caregivers involved in the client’s care, helping to resolve any disputes that may occur. \n
- Educate clients and families about available resources, support groups, and community programs that can foster social engagement and improve overall well-being. \n
- Maintain accurate and comprehensive records of client assessments, care plans, progress, and communication in accordance with ethical and legal standards. \n
- Stay updated on best practices in social work and hospice care, ensuring compliance with organizational policies and state regulations. \n
This position primarily focuses on seniors in hospice care who often face complex emotional and physical challenges. Our clients may be dealing with terminal illnesses, end-of-life issues, or transitions into assisted living facilities. Qualifications\n- \n
- A Master’s degree in Social Work from an accredited institution. \n
- Current and valid state licensure as a Licensed Clinical Social Worker (LCSW). \n
- A minimum of two years of relevant social work experience, preferably in a geriatric or community center setting. \n

- Strong assessment, problem-solving, and crisis intervention skills. \n

The salary for this role ranges from $60,000 to $100,000 annually, commensurate with experience and qualifications.
