We are seeking a Field Nurse Case Manager to cover the Northern Bay Area, CA, and surrounding areas . This is a home-based position with travel, up to 2 hours one-way, to doctors’ offices, hospitals, and other locations. The individual is responsible for medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager collaborates with insurance carriers, medical providers, attorneys, employers, and employees, monitoring the injured worker’s progress and reporting results to the employer and insurance carriers.

Qualifications:

• Current, unencumbered Registered Nurse (RN) license in California
• Experience in workers’ compensation case management is preferred
• Certifications (CCM, CRRN, CDMS, CRC) are strongly preferred and must be obtained within two years of hire
• Skills: organization, adherence to timelines, PC and technology proficiency, effective communication (written, verbal, interpersonal)
• Ability to work autonomously with minimal oversight
• Patient education skills
• Valid driver’s license and good driving record
