In-House Subacute Rehabilitation

Radiant Nursing and Rehabilitation in Palatka, FL

Position Overview
Radiant Nursing and Rehab in Palatka, FL is seeking a skilled and compassionate  Occupational Therapist (OTR)to join our  in-house subacute rehabilitation team . This PRN offers the opportunity to lead patient-centered care, helping residents recover and regain independence following illness, injury, or surgery.

Pay starts at $50/hr and increases based on experience level.

Key Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans
• Deliver evidence-based interventions to improve ADLs, functional mobility, strength, and cognition
• Supervise and collaborate with Certified Occupational Therapy Assistants (COTAs)
• Monitor patient progress and modify treatment plans as appropriate
• Maintain accurate, timely documentation in compliance with regulatory standards
• Collaborate with an interdisciplinary team including PT, SLP, nursing, and physicians
• Provide education to patients, families, and caregivers

Qualifications

• Current  Florida Occupational Therapist (OTR) license
• Graduate of an accredited Occupational Therapy program
• Experience in  skilled nursing, post-acute, or subacute rehabilitation preferred
• Strong clinical reasoning and communication skills
• Leadership ability and team-oriented mindset
