Job Summary:
Seeking an experienced RN specializing in Case Management to provide comprehensive patient care coordination in a hospital setting. The chosen candidate will facilitate patient transitions, ensure compliance with care plans, and support clinical staff to optimize patient outcomes.

Responsibilities:

• Coordinate patient care activities across multidisciplinary teams to ensure seamless treatment flow.
• Assess patient needs and develop individualized care plans in collaboration with medical staff.
• Monitor patient progress and adjust care plans as necessary to meet clinical objectives.
• Educate patients and families on post-discharge care, resources, and support services.
• Document clinical interventions, assessments, and patient responses accurately in electronic health records.
• Comply with hospital policies, regulations, and accreditation standards related to case management.

Qualifications:

• Hold a valid Registered Nurse license with at least 1 year of clinical experience.
• Demonstrate strong case management skills within a hospital environment.
• Exhibit excellent communication and patient advocacy abilities.
• Possess knowledge of healthcare laws, regulations, and documentation protocols.
• Ability to work effectively in a fast-paced, team-oriented setting.
• Prior experience in patient discharge planning and resource coordination preferred.
