California Licensed LVN Case Manager

A company is looking for an LVN Case Manager. Key Responsibilities Assess member needs across various health domains and develop care plans Coordinate healthcare benefits and advocate for clients with healthcare professionals Maintain patient health records and complete documentation within established timeframes Required Qualifications Active, unrestricted Licensed Vocational Nurse (LVN) license in California 2+ years of experience in care management or related fields Knowledge of government and community resources related to social determinants of health Willingness to work Monday-Friday with potential for evenings and weekends Bilingual (English/Spanish) preferred
