Home Health Physical Therapist

Schedule
- Shift: 8-hour day shifts only

Assignment Details
- Patient Mix: Adults / Geriatrics
- Visit Expectation: May see up to 6 patients/day; 30 productivity points standard
- Travel: Occasional radius up to ~60 miles

Requirements
- Required: 1 year of skilled home health experience
- Required: Licenses in both OR and WA
- Required Certifications: BCLS / BLS
- Required Clinical Skills: Home Health assessments and OASIS documentation, admissions, lifting and transfers
- Neurologic requirement: Admissions experience required
- Preferred: Wound care, PTA experience

Roles & Responsibilities
- Conduct patient assessments and develop individualized treatment plans.
- Perform home health visits and provide rehabilitation interventions consistent with patient needs.
- Document care and maintain accurate OASIS records as mandated by regulations.
- Communicate effectively with patients and their families regarding treatment progress and needs.
- Coordinate with interdisciplinary team members to deliver comprehensive care.
- Manage ordering of supplies and scheduling of appointments as needed.

Compliance / Notes
- Confirm RTO/scheduling restrictions with the hiring manager.
- Orientation and pre-start modules may be required.
