Licensed Practical Nurse (LPN) – Long-Term Care (LTC)

Location - Vestal, NY

Duration - 13 weeks

Position Overview:
Seeking a dependable and compassionate Licensed Practical Nurse (LPN) with long-term care experience to provide high-quality nursing care in a skilled nursing facility. The ideal candidate will be experienced in medication administration, resident assessment, and working collaboratively with interdisciplinary care teams.

Required Qualifications

Active LPN License
Minimum 1 Year of Long-Term Care (LTC) Experience
Ability to Successfully Pass a Medication Administration (Med Pass) Test
Ability to Successfully Complete Clinical Observation Requirements

Key Responsibilities

Administer medications and treatments safely and accurately
Perform medication passes in accordance with physician orders and facility policies
Monitor residents and report changes in condition to the appropriate healthcare providers
Complete timely, accurate, and compliant clinical documentation
Deliver direct nursing care to residents in a long-term care setting
Assist with resident assessments and care planning
Supervise and support Certified Nursing Assistants (CNAs) on assigned units
Ensure compliance with all regulatory and facility standards
