Compensation & Schedule:
$22.00 – $25.00 per hour (based on experience)
Performance-based bonuses (including census incentives)

Key Responsibilities:
Administer medications and treatments per physician orders
Monitor residents and report changes in condition
Complete accurate documentation and incident reports
Follow up on physician orders, labs, and care plans
Assist with admissions and service plan implementation
Supervise and support caregiver staff
Maintain compliance with WV assisted living regulations
Follow infection control and safety protocols
Communicate effectively with residents, families, and team
Qualifications:
Active LPN license in West Virginia (required)
Graduate of an accredited nursing program
Experience in assisted living or long-term care preferred
Ability to work independently and make sound clinical decisions
