As a Home Health Registered Nurse (RN), you will work collaboratively with a healthcare team to develop treatment plans tailored to each patient. You will have the opportunity to creatively utilize resources available in patients' homes, educate patients and their families, and adapt care strategies to meet the changing needs of those you serve.
Service AreasThis role covers the following counties in Illinois: Clinton, Washington, Perry, Marion, Jefferson, Franklin, Wayne, Hamilton, and Edwards.
CompensationSalary Range: $32.77 - $52.43 per hour
Compensation is determined based on various factors, including relevant experience and market conditions. The stated pay range reflects what we reasonably anticipate offering at the time of posting, but actual pay may vary in accordance with applicable laws.
Key Responsibilities- Evaluate and coordinate discharge planning needs in collaboration with the healthcare team.
- Conduct point-of-care testing as per established protocols.
- Engage with physicians to discuss care plans and any changes in patient status, providing regular updates and documentation in compliance with regulations.
- Coordinate with multidisciplinary team members and external agencies to ensure effective patient care and education throughout the discharge process or end-of-life care.
- Complete admission documentation and verify insurance details, ensuring timely medical assessments are recorded in the clinical system.
- Deliver skilled nursing care as outlined in the care plan, including vital sign monitoring, wound care, IV therapy, and other medical procedures.
- Document all visits, treatments, and care accurately and promptly, adhering to regulatory standards.
- Collaborate with patients, families, and the healthcare team to create a personalized care plan that addresses individual patient needs.
- Respond to inquiries from patients and families, providing follow-up as necessary.
- Maintain a vigilant and safe working environment.
- Perform additional duties as assigned.
- Education: Graduate from an accredited nursing program or equivalent educational background for licensing.
- Experience: No prior experience required.
- Additional Requirements: Proof of motor vehicle liability insurance that meets state minimums upon hire and upon request.
- Frequent verbal communication and active listening.
- Regular standing and walking.
- Occasional lifting and carrying of objects weighing up to 50 lbs.
- Frequent use of vision for close and distant tasks, as well as color differentiation.
- Regular driving and data entry tasks.
- Occasional patient handling and physical movements such as bending and twisting.
Applicants must hold the following certifications based on their state of employment:
- Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA)
- Registered Nurse (RN) license as per state regulations
- Valid Driver's License and Motor Vehicle Report (MVR)
