What Your Day Looks Like at Custom:

As part of our dedicated team, you'll provide hands-on personal care to patients, ensuring their comfort, dignity, and well-being by:

Assisting with personal hygiene needs like bathing, oral care, and grooming.
Supporting daily activities such as dressing, toileting, and ambulation.
Maintaining a clean and orderly living environment for patients.
Preparing and serving nutritious meals, assisting with feeding if needed.
Following personalized care plans in collaboration with nurses and interdisciplinary teams.
Observing and reporting significant changes in the patient's condition to the assigned nurse.
Documenting care in line with organizational standards and maintaining professional conduct.
Participating in orientation, staff meetings, and training programs (e.g., QAPI, Emergency Preparedness).

Note: This job description provides an overview of responsibilities and may include other duties as assigned.

Who You Are:

Certified Professional : Valid certification through the state (required).
Experienced Caregiver : At least 1 year of certified nursing experience.
Age Requirement : Minimum of 18 years old.
Detail-Oriented : Ability to read, follow written instructions, and document care accurately.
CPR Certified : Current certification is required.
Compassionate : Demonstrates tact, patience, and a commitment to excellent hygiene and professional care.
