Georgia Hospice Care is seeking a Clinical Liaison to build and maintain relationships with hospitals, physicians, social workers, and other referral sources to facilitate access to hospice services. You will educate providers and families about eligibility, coordinate referrals, and advocate for patients and families facing serious illness. You will collaborate with interdisciplinary teams to ensure seamless transitions and exceptional patient experiences, while representing the organization at #J-18808-Ljbffr Georgia-Hospice-Care-2
