Case Manager/ Registered Nurse

Houston Methodist seeks a Case Manager/Registered Nurse to coordinate comprehensive care for patients across the continuum. In this role, you will conduct clinical and psychosocial assessments, develop and manage individualized care plans, and lead safe, timely discharge planning. You’ll collaborate closely with physicians, social workers, and the interdisciplinary team to optimize resource utilization, reduce readmissions, and support patient and family education.
