Medical Social Worker

Regional Medical Social Worker Provides patient care services across regional offices, conducting initial psycho-social assessments and developing individualized care plans. Identifies and reassesses social, emotional, financial, and environmental factors impacting treatment outcomes. Communicates effectively with healthcare teams and utilizes community resources to support patient needs. Educates patients and families on resource utilization and discharge procedures. Documents care data and follows up on treatment goals. Performs other duties as assigned. Qualifications Master’s degree in social work from an accredited institution, eligible for North Carolina certification. Baccalaureate degree in social work, psychology, sociology, or related field, with at least one year of healthcare social work experience. Preferably experienced in home health care. Strong interpersonal and communication skills within multidisciplinary teams. Willing to travel and adapt to working conditions as needed. #J-18808-Ljbffr
