Clinical Reentry Liaison (Grant Funded)

Job Title Clinical Reentry Liaison (Grant Funded) Requisition ID 00139028 Organization Sheriff's Department Posting Details Full-time, A.M. P.M. Salary: $81,603 anually Location: Cook County Type: Civilian (non-sworn), non-merit ranked Responsibilities Manage the day-to-day programmatic components of clinical initiatives including screening, assessment, intake, treatment planning, case management, care coordination, documentation, participant lists and tracking systems, clinical supervision, and oversight of staff to ensure the program operates effectively. Operate from a trauma-informed approach during clinical and professional engagement. Assess participants for clinical, socio-emotional and reentry service needs; focus on diagnostic and functional evaluations and make recommendations for treatment goals and plans; collect information through interviews and observations. Conduct crisis needs assessment, crisis intervention, and crisis safety planning; provide de‑escalation and emergency care coordination, including assistance with civil commitment and medical placement. Provide focused intervention of participants; offer consultation and training to staff to enhance their skill in managing impacted participants. Oversee the implementation of new programming opportunities and collaborate with supervisors to build and maintain relationships with community partners. Communicate regularly with Sheriff’s Office leadership and staff to ensure coordinated release plans are executed appropriately. Collaborate with external partners regarding returning citizen referrals to set and execute coordinated releases. Establish a supportive, motivational, therapeutic relationship; plan, organize, and lead structured interventions to support behavioral changes and healthier life choices. Guide participants in skill development for dealing with specific problems; assist with mental health and chemical dependency issues; serve as a role model and coach for problem solving, decision-making, and conflict resolution. Evaluate the effectiveness of treatment plans in correlation with participant progress toward identified goals. Act as an advocate for the participant and the department in coordinating services with treatment partners and legal system stakeholders. Complete and maintain service-related paperwork, including reports, care plans, and progress reports per state and federal requirements. Track day-to-day operations to maintain or improve service quality and ensure grant compliance. Maintain records and data to generate monthly, quarterly, and annual reports and support grant reporting. Propose, plan, and implement new initiatives to strengthen services provided to participants. Provide consistent feedback to supervisors on improving program processes, operations, and outcomes. Assist in implementing strategies to increase access to programs while considering the needs of the justice population and community resources. Complete all additional duties as required by the executive team. Qualifications Possession of a master’s or higher degree in Counseling, Psychology, Social Work, Social Sciences, or a closely related field from an accredited institution. Active state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC). Four (4) years of full-time paid experience in behavioral health services and/or social services. Proficiency in Microsoft Office Suite. Ability to work in professional office, a jail/prison, and in the community. Ability to work any shift assigned, including days, afternoons, midnight shifts, holidays, weekends, mandatory overtime, and after‑hours on‑call shifts as required. Preferred Qualifications Certification as a Certified Alcohol and Other Drug Abuse Counselor (CADC). Experience working with additional populations such as housing/homelessness, substance use treatment, behavioral health, community corrections, or domestic violence settings. Two (2) or more years of clinical supervision experience. Six (6) or more years of full-time professional experience in community mental health or hospital settings performing counseling and assessments.
