The LPN will deliver resident-centered nursing care, including medication administration, treatments, vital sign monitoring, and ADL support for short-term rehab, long-term, and memory care residents. Working within a collaborative, family-like team, the LPN will participate in care planning, maintain accurate documentation, uphold infection control and safety standards, and communicate effectively with residents, families, and the interdisciplinary team to support dignity, comfort, and quality of life.
