What You Will Do

• Conduct functional physical therapy evaluations and develop personalized plans of care tailored to residents in assisted living facilities.
• Deliver focused, one-on-one treatments emphasizing functional mobility, gait, balance, strength, and fall mitigation.
• Collaborate directly with community staff, physicians, caregivers, and families to support whole-patient wellness.
• Maintain clean, compliant, and timely documentation using our streamlined, mobile-friendly EMR system.
• Help shape company culture as our small, patient-first team continues to expand.

Who You Are

• Licensed Clinician: Valid (or pending) State Physical Therapy License and graduate of an accredited Physical Therapy program (DPT or MPT).
• All Experience Levels Welcome: Prior experience in geriatric, home health, or ALF settings is a plus, but passion, coachability, and cultural alignment matter most. Comprehensive mentorship is provided for newer clinicians.
