Licensed Practical Nurse

Overview Licensed Practical Nurse (LPN) providing quality patient care and performing technical skilled care in the patient’s home. Responsible for delivering clinical services within state practice regulations and supporting the home health care team. Primary Responsibilities Provide high quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other health‑care team members; follow all applicable infection control procedures. Document patient visits thoroughly and completely in the electronic medical record and the agency’s electronic visit verification program, per regulatory and payer requirements. Actively participate in the agency’s care coordination process, reporting patient needs and condition changes, attending case conferences, and communicating effectively with patients, families, physicians, clinicians, agency management, and case coordinators. Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client. Required Qualifications Current and unrestricted LPN licensure in the state of practice. Current driver’s license and vehicle insurance, access to a dependable vehicle, or reliable public transportation. Preferred Qualifications Current CPR certification or ability to complete within 90 days of hire. Ability to manage multiple tasks simultaneously. Capability to work independently. Good communication, writing, and organizational skills. Ability to work flexible hours as required to meet identified client needs. Compensation $46,564 - $69,846 annual total cash target pay $22.39 - $33.58 per visit point $26.86 - $40.30 hourly rate Annual total cash compensation assumes full‑time employment (40 weekly hours) at full productivity and follows the range above. Compensation includes earnings from per‑visit point pay and hourly pay, adjusted for local labor markets, education, work experience, producti­vity, and performance. Roles receive two types of compensation depending on the work performed. Visit‑based pay is based on productivity points assigned to each visit type. Non‑visit activities, such as orientation, are compensated at the hourly rate.
