Responsibilities: Providing crisis intervention, counseling, and support to residents and their families Developing and implementing individualized care plans that address the social and emotional needs of our residents Coordinating with our interdisciplinary team and other healthcare professionals to ensure comprehensive, continuous care and optimal outcomes Conducting social histories and assessments to identify individual needs and develop strategies for addressing those needs Maintaining accurate and up-to-date records of resident care and services Collaborating with other departments to ensure effective communication and coordination of care Requirements: A minimum Bachelor's degree in Social Work (BSW). However, a Master of Social Work (MSW) is highly preferred. Minimum of 1 year experienced as a full‑time, supervised social worker in a health care setting working directly with individuals. Experience in geriatrics or a skilled nursing facility (SNF) is highly preferred. Must hold an active license or certification (e.g., LSW, LCSW). Strong interpersonal and communication skills. Ability to work in a fast‑paced environment and prioritize multiple tasks and responsibilities. Ability to maintain confidentiality and handle sensitive and confidential information.
