$5,000 Sign-On Bonus!

Hospice RN Case Manager — Where Support Meets Impact

Lower patient-to-nurse ratios for meaningful, unrushed hospice care
Strong interdisciplinary team support—collaborative care with Social Work, Chaplaincy, Aides, and providers, so you’re never practicing alone
Leadership that understands hospice nursing
Dedicated internal after-hours triage team to support nurses outside of regular business hours
Homecare Homebase (HCHB) EMR for streamlined documentation

Office Location: 1028 S. Main Street Suite C Fond du Lac, WI 54935
Coverage area:  South Fond du Lac areas - Columbus/Fall River/Beaverdam/Mayville

Schedule:  Monday-Friday 8AM-5PM
