Licensed Practical Nurse II – Wrentham Developmental Center Location: Wrentham, MA. Job Responsibilities Provide exceptional nursing care and oversight, including evaluation of behavioral and episodic health needs through assessment, continuous observation, and evaluation of interventions as part of the behavioral/health care plan. Administer medications and treatments in accordance with medical provider orders and established policies and procedures. Safeguard the security of controlled substances per policies to prevent tampering or diversion. Monitor equipment functionality and ensure availability of supplies and medications. Perform all clinical tasks in adherence to acceptable standards of nursing practice to ensure safe and effective care. Act as an advocate for individuals and demonstrate respect and positive regard for individuals with developmental disabilities. Provide support to individuals on the behavioral unit who exhibit challenging and extreme behaviors. Monitor health and safety, adhere to rules and regulations, and communicate with nursing and DSW supervisors regarding behavioral and health concerns. Seek supervisory assistance or guidance when needed including administrative issues and evolving clinical/behavioral presentations. Assist in planning, coordinating, and implementation of care for acutely ill or injured individuals; function as a first responder in emergencies or crises. Complete daily reports and document incidents occurring during the shift. Preferred Qualifications PABC Certified and/or PABC trainer Experience working with challenging behaviors. Knowledge of PBS and its implementation across settings. Ability to remain calm in high‑stress environments. Excellent time‑management and communication skills. Develop and maintain respectful, caring, tolerant, ethical, and empowering relationships with staff and individuals. Professional and appropriate communication with others. Interface appropriately with other staff and supervisors. Willingness to debrief staff after stressful situations. Proficient with Microsoft operating systems. Provide leadership and direction to direct care staff while responding to emerging situations. Ability to assist/lift individuals with physical disabilities and/or limited mobility. Proficiency in reading and comprehending English. Skill in cultivating a team atmosphere. Knowledge of standard nursing policies, procedures, methods, and techniques. Special Requirements LPNs applying for this assignment must complete the Proactive Approaches to Behavioral Challenges (PABC) training within 6 months of assignment. LPNs that cannot complete the training will be ineligible. The selected candidate must acquire and maintain PABC certification and current Cardiopulmonary Resuscitation (CPR) certification. Position Requirements Current, active license to practice as an LPN in the Commonwealth of Massachusetts. Agency Requirements Current and valid Class D Motor Vehicle Operator’s License. Health Care Provider Basic Life Support (BLS) certification. Other Employment Conditions Assignments may require 24/7 nursing coverage. Employees in these positions must be able to perform mandatory overtime as circumstances dictate. This position is designated an “EMERGENCY” position under the Department of Developmental Services Emergency Plan; during a state of emergency, the employee will be required to report to work. Title 101 CMR 23.00 requires agency staff to have received COVID‑19 and Influenza vaccinations, or to have taken required mitigation measures, to prevent viral infection and transmission.
