Responsibilities
- Provide and supervise day-to-day nursing care for residents during your shift, ensuring compliance with applicable federal, state, and local standards and regulations.
- Administer medications and treatments as prescribed; monitor and document resident responses and changes.
- Assess residents’ health status, identify evolving needs, and communicate findings to the care team and supervisors.
- Implement rehabilitative and restorative nursing plans to support residents’ functional goals.
- Supervise and support nursing assistants and other care staff; maintain accurate documentation and uphold infection control practices.
- Participate in quality improvement activities, safety initiatives, and care planning meetings as assigned.

Requirements
- Active LPN license in Colorado.
- Current BLS certification; IV certification preferred for LPNs.
- Minimum 3 years of experience in a hospital, skilled nursing facility, or comparable setting; training in rehabilitative and restorative nursing.
- Ability to work in a memory care–focused long-term care environment; Colorado CAPS check required.
- Must have access to reliable transportation to accommodate rural Gunnison location.
