Nurse Practitioner

We are seeking a dedicated Full-Time Nurse Practitioner to provide primary care to elderly participants within a community-based program. Key Responsibilities Include Performing physical assessments and developing individualized care plans for new and existing participants. Evaluating physical complaints, providing appropriate treatments, and educating participants and caregivers. Collaborating with an interdisciplinary team to coordinate comprehensive care and discharge planning. Overseeing residents in skilled nursing facilities and coordinating with primary care physicians and medical directors. Conducting semi-annual reassessments and integrating primary treatment plans into overall care. Qualifications Required Licensed Registered Nurse (RN) and certified Nurse Practitioner recognized by the State Board of Nursing. Graduate of a professional nursing school with at least one year of experience working with the elderly population. Preferred Qualifications Include Geriatric Nurse Practitioner certification with prescriptive authority. Two or more years of geriatric healthcare experience. The role requires: onsite work with occasional travel, participation in a team-oriented environment, and a focus on delivering compassionate, patient-centered care. #J-18808-Ljbffr
