What You'll Do:

• Effectively evaluate, screen, and rehabilitate patients with physical functioning disorders and wound care needs.
• Develop personalized care plans following all regulatory and clinical standards.
• Administer a variety of procedures including manual therapy, ambulation, therapeutic exercises, modalities, supportive devices, and wound care.
• Monitor patient progress and adjust treatments accordingly, documenting all interventions accurately.
• Collaborate with patients, families, caregivers, and healthcare team members to optimize recovery outcomes.
• Provide clear communication regarding patient progress, barriers, and treatment strategies.
• Support discharge planning and compile comprehensive service summaries.
• Maintain precise documentation in electronic health records and participate in team meetings, trainings, and reviews.
• Supervise and support rehabilitation aides, students, and assistants, ensuring quality and compliance.
• Uphold professional standards and ethical practice in line with APTA guidelines and state regulations.

Required Skills:

• Valid State Physical Therapist license.
• Proven experience in outpatient or LTC settings.
• Strong knowledge of rehabilitation techniques, wound care, and patient assessment.
• Excellent communication, teamwork, and documentation skills.
• Ability to work independently and support a team-focused environment.

Nice to Have Skills:

• Experience with modern rehabilitation equipment and modalities.
• Bilingual abilities or additional language skills.
• Familiarity with electronic health record systems.

Preferred Education and Experience:

• Bachelor's or Doctorate degree in Physical Therapy.
• Prior experience in long-term care facilities or similar settings preferred.
