Metro Rail Outreach Case Manager — Housing & Services Advocate
PATH
PATH is seeking an Outreach Case Manager in Los Angeles, CA to conduct daily street outreach and engage clients experiencing homelessness within the Metro transit system. You will assess needs, connect clients to housing, health and supportive services, and coordinate with local partners to facilitate referrals and transportation assistance. You will work on the Metro Rail Outreach Team, collaborating with community providers to build individualized service plans, document outcomes, and advocate #J-18808-Ljbffr PATH
