Assists clients in navigating peer support, wellness services, and other resources within the behavioral health system of care, as well as other health systems and community-based supports. Collaborates with the care team to connect clients with resources such as housing, food assistance, education and vocational opportunities, recreation, spiritual supports, mentors, and mental and physical health services. Encourages positive social interaction and engagement with peers, providing constructive feedback on appropriate behaviors while serving as a positive role model. Additionally, advocates for clients’ needs and promotes self-advocacy to support their independence and overall well-being.Division/Program Overview:Population Served: Serving adults, 18 plusFacility & Setting: Outpatient community-based services and support. Delivers client services throughout Fresno County within designated county limits.
