Good Seed Community Development Corporation is looking for an ECM Case Manager based in San Diego, California. This position is focused on community outreach and engagement, eligibility determination, and conducting assessments for the Enhanced Care Management program. Candidates should have a Bachelor’s in social services, preferably with 2-3 years of experience in community health or social services, strong case management skills, and the ability to work in an interdisciplinary team. Proficiency in Microsoft Office and familiarity with Electronic Health Records are preferred.
