PATIENT INFORMATION • Patient Demographics: Geriatric/frail elderly • Patient Volume: TBD • Admissions: As Needed • Rounding included: Yes • Phone Consults: As Needed • Case Mix: Primary care, chronic disease management, preventive care, and transitional care

FACILITY INFORMATION • Collaborative environment with physicians, nursing, therapy, and support staff

REQUIRED PROCEDURES/DUTIES • Comprehensive geriatric assessments • Chronic disease management • Preventive care screenings • Medication management • Transitional care and discharge planning

JOB REQUIREMENTS • Active NC License • NCCPA certification • DEA • BLS • Clean Record (highly preferred) • 2 years PA experience • Experience with geriatric or frail elderly populations (preferred)
