The Healthy Families Family Support Specialist 1. Initiates and maintains regular long-term contact and support with families for five years with each family. Ensures services are provided in the family's home. Ensures each visit lasts about one hour. Each visit should maintain family-centered and strengths-based approaches at all times. 2. Ensures the quality of home visiting services helps families achieve core Healthy Families America model quality, fidelity, and success for each family. Each family will receive evidence-based interventions that strengthen the parent-child relationship by using evidence-based curriculum and activities on home visits. Each family will receive evidence-based interventions that promote healthy childhood growth and development by using evidence-based curriculum and activities on home visits. Each family will receive evidence-based interventions that reduce risk factors and increase resiliency factors by using evidence-based curriculum and activities on home visits. Maintains strong knowledge of evidence-based curriculum and quality interventions. 3. Ensures caseload and home visiting rate is maintained in accordance with Healthy Families America productivity standards: Maintain caseload of 15 level one families or a weighted caseload of no more than 30 families. Ensure the home visitation completion rate of 85% or above is maintained at all times by conducting home visits at an interval based on each family's level of care. Ensure productivity is completed at 100% at all times by completing home visits, brief home visits, attempted home visits, office visits, and extra home visits. 4. Provides direct family-centered case management to families to ensure needs are met and case management milestones are maintained in accordance with the Healthy Families America standards for case management, including: All families will achieve obtaining a medical home in accordance with the Healthy Families America model timetable. All well-child exams will be maintained according to the American Academy of Pediatrics Schedule of Well-Child Visits. All children will maintain completed immunizations in accordance with the American Academy of Pediatrics Schedule of Childhood Immunizations. All children will receive the Ages and Stages Questionnaire completed in accordance with the required Healthy Families America model timetable; all children presenting with a developmental delay will receive referrals for early intervention services if the family consents. All children will have the Ages and Stages Questionnaire ? Social and Emotional completed in accordance with the required Healthy Families America model timetable; every child whose scores indicated additional mental health assessment will be referred to an infant mental health assessment or home visiting service plans will reflect interventions to address this need if the family consents. All prenatal and postnatal women will have Depression Screens completed in accordance with the Healthy Families America model timetable; all screens that indicate a need for mental health assessments will result in a referral for additional mental health care if the woman consents. All families will have Domestic Violence screens completed in accordance with the Healthy Families America model timetable. All families will have a Cheers Check In (Parent-Child Interaction tool) completed in accordance with the Healthy Families America model timetable. All families will receive appropriate referrals to community resources based upon family needs. When child safety concerns arise, workers will immediately notify their supervisors and will make referrals to the Child Abuse Hotline or DCS when deemed necessary. 5. Maintains documentation of services, quality measurement tools, and tracking forms in accordance with the Health Families America model, including: Ensuring home visit notes are completed and turned into supervisor within 2 business days. Recording well-child exams and immunizations in the chart within 1 months of the occurrence. Completing and turning into supervisor the monthly service records and progress notes by the 3 rd of each month. Utilize the Tennessee Department of Health's REDCAP database to input required forms and screenings tools within 10 business days of the occurrence. 6. Maintains Family Service Plans for all families in accordance with Healthy Families America model standards, including: The Family Service Plan is completed within 14 days of entry into services for all newly enrolling families. The Family Service Plan is updated at least monthly for all enrolled families. 7. Maintains completion of required supervision, including: Ensuring attendance of 1.5 to 2 hours of weekly reflective supervision each week. Willingness to engage in and building reflective capacity. 8. Completes required trainings in accordance with Healthy Families America model standards, including completing: Initial Foundations for Family Support Core Training. Required HFA Wrap-around training in the first year Annual Child Abuse and Neglect training. Annual Diversity Equity and Inclusion training. Annual McNabb Center Relias trainings, CPR and First Aid, and Handle with Care trainings. 15 hours of Infant Mental Health Training annually to maintain Endorsement. 9. Achieve and maintain Infant Mental Health Endorsement through the Tennessee Association of Infant Mental Health, including: If hired without an Endorsement, will submit application on Endorsement Application System, uphold Ethics Code, achieve mandatory references, and complete and submit 30 hours of required infant mental health training within first year of employment. Once Endorsement is achieved or if hired with an endorsement, will maintain endorsement every year but completing and submitting 15 continuing education hours and updating the Code of Ethics review process annually. 10. Takes an active role in ensuring the McNabb Center completes and maintains Healthy Families America Accreditation, including: Familiarizing self with the more than 150 required Policies and Procedures. Ensuring all documentation is maintained for accreditation. Ensuring trainings are maintained. Ensuring home visiting rates are tracked and home visits are completed. Completed accreditation interview. 11. Utilizes Reflective Practices in all aspects of work, which includes but is not limited to: Using the 7 Core Healthy Families America Reflective Strategies with all families during home visiting services. Maintaining interest in and the ability to see things from multiple points of view. Using an ecological perspective when working with families. Maintaining up-to-date knowledge of infants, toddlers, and attachment and the ability to their relationships with their caregivers. Using self-awareness by being clear on one's own values and triggers and how these things influence their perceptions and interpretation of family needs and the impact on families. Maintaining a balanced and realistic view of relationships including positive and negative aspects. Talking about and thinking about relationships and personal feelings that arise during supervision. Considering another person's feelings, wants, needs, and states of mind so interactions and responses can be intentional. Ensuring if ruptures occur with a family, they are repaired. Taking responsibility for their own contributions and reactions to difficult situations. Tolerating ambiguity. Recognizing and taking action when oneself needs to be put in a more balanced state. This job description does not constitute a written or implied contract of employment. COMPENSATION: Starting salary for this position is based on relevant experience and education. The starting salary for high school prepared applicants is $17.21/hr. The starting salary for bachelor's level applicants is $18.29/hr. QUALIFICATIONS - Healthy Families Family Support Specialist Education / Knowledge: Bachelor's degree in Social Work, Psychology, or related field preferred. High school diploma and lived experience required if not Bachelor's Qualified. The Association of Infant Mental Health in Tennessee (AIMHITN) Infant Mental Health Endorsement preferred. Required to achieve AIMHITN Infant Mental Health Endorsement within the first year of employment if hired without endorsement. Training and knowledge in infant and early child development and parenting skills required. Experience : One-year experience providing evidence-based home visiting services preferred. Experience working with young children and families required. Physical/Emotional/Social -Skills/Abilities: To perform this job successfully, an individual must have excellent communication skills with colleagues and clients. Must be able to build quality and caring relationships with clients where clients feel supported and heard. Must be able to maintain a trauma-informed approach when serving families. Must maintain appropriate boundaries with clients and colleagues. Willingness to engage in building reflective capacity, must be able to manage a flexible schedule and multiple tasks. Must be able to use reflective practices in working with families. Must be able to work with diverse populations in culturally sensitive ways. Must have an F endorsement and be able to drive for work-related duties. Must be able to utilize a dependable vehicle for home visitation services.
