Behavioral Health Therapist II, Primary Care at Darley Green

Job Details ChristianaCare’s Integrated Behavioral Health Department is seeking a compassionate, dependable, and detail‑oriented Behavioral Health Therapist II who demonstrates strong clinical expertise, trauma‑informed care, and has the ability to build trusting therapeutic relationships with a diverse patient population. The role supports our Darley Green Primary Care location at Claymont, DE. Work Schedule Monday- Friday: 8:00am-4:30pm No weekends or holidays This is a hybrid role with four (4) days onsite, and one (1) remote day option available. Remote days are subject to change and may be adjusted on an departmental needs. Key Responsibilities Provide psychological care appropriate for the primary care population, including assessments, interpretation of information, and culturally specific care. Act as a consultant and liaison for the Darley Green department, meeting acute patient needs and providing outpatient psychotherapy as appropriate. Coordinate transition of care to community settings as necessary; maintain knowledge of community resources. Deliver behavioral health consultation to the clinical team. Collaborate with interdisciplinary team and community partners. Develop group support and therapy activities appropriate for the primary care population. Contribute to staff development by providing consultation, education, and best practice alternatives. Maintain connection and consultation with the Department of Psychiatry and Behavioral Health. Complete mandatory CCHS education and maintain certification in area of specialty. Maintain established productivity levels. Maintain patient confidentiality and patient rights. Identify opportunities to improve performance and patient psychosocial needs; plan and implement programs. Provide virtual coverage for primary care sites without a placed BHC or with a part time BHC not present. Respond to BH suicide triage line and specialty practice providers with patients presenting in crisis. Attend weekly BHC meetings and perform related duties as required. Qualifications Master’s degree qualifying for state licensure or certification. Required licensure: Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor of Mental Health (LPCMH), or Licensed Marriage and Family Therapist (LMFT). Delaware state licensure preferred. Minimum one year of experience providing direct therapeutic care preferred.
