Physical Therapist
Job Summary:
GEM Transitional Care Center is seeking a compassionate and experienced Physical Therapist to join our dynamic team! As a Physical Therapist, you will play a vital role in helping our patients regain their independence and mobility after an illness, injury, or surgery. Responsibilities:
Conduct initial patient evaluations to identify physical therapy needs and develop individualized treatment plans
Provide one-on-one physical therapy sessions to patients, focusing on strengthening, mobilizing, and educating them on proper techniques
Develop and implement creative and effective treatment plans to achieve patient goals and outcomes
Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient results
Participate in interdisciplinary team conferences to discuss patient progress and adjust treatment plans as needed
Maintain accurate and detailed patient records, including progress notes and treatment plans
Stay current with industry developments and best practices by attending continuing education courses and workshops
Requirements:
Physical Therapist degree from an accredited institution
Current Board Certification in Physical Therapy (BPTh)
Minimum of 0 years of experience working as a Physical Therapist
FULL_TIME commitment to providing exceptional patient care
